Fidaxomicin (fidaxomicin, Dificid) Fidaxomicin, developed by Optimer, was approved by the FDA on May 27, 2011. Fidaxomycin is a macrolide antibiotic with a novel mechanism of action, mainly by inhibiting the bacterial RNA polymerase to produce rapid anti-infection of Clostridium refractory (CDI). Its treatment of CDI is superior to existing drugs. The safety and efficacy of fidaxomicin have been verified by 2 large multicenter, randomized, double-blind, active drug controlled clinical trials. Common adverse reactions of fidaxomicin include nausea, vomiting, headache, stomach pain and dysentery.

Fidaxomycin is a fermentation product of Actinoplans deccanensis or Dactylosporangiumaurantia Chemicalbookcum, which belongs to a new 18-membered macrolide compound. The molecular weight is 1058.04, and the molecular formula is C32H74C12O18. It can generate the still active metabolite OP-1118 in vivo. This product inhibits bacterial proliferation by binding to RNA polymerase. Most RNases are composed of five subunits, which are composed of (α2ββ'ω) as the core catalytic enzyme, and the other is the б subunit, which is responsible for promoter recognition. Due to the б in CD The subunit is different from other bacterial strains, so far, no cross-resistance with other antibacterial drugs has been found in in vitro studies. Fidamicin and its major metabolite OP-1118 are substrates for the P-glycoprotein efflux transporter, which is present in the GI tract. Studies have evaluated the interaction of fidamicin with P-glycoprotein substrates and their inhibitors, as well as with CYP450 isozymes 3A4, 2C9 and 2C19. Study drugs included cyclosporine, digoxin, midazolam, omeprazole and warfarin. Although co-administration of cyclosporine with fidamicin nearly doubled the maximum plasma concentrations of fidamicin, these changes were not clinically meaningful because concentrations were still in the ng/mL range. There was no change in the pharmacokinetics of fidamicin for concomitant use of the other drugs mentioned above. Based on current research, no dose adjustment is required when fidamicin is administered concomitantly with drugs that alter P-glycoprotein efflux transporters and cytochrome P450 isoenzymes.
