Roxadustat is a novel hypoxia-inducible factor (HIF)-prolyl hydroxylase (PH) enzyme inhibitor. It stabilizes HIF, inhibits its degradation, activates the transcription of related genes, and produces corresponding physiological responses. Increase the concentration of erythropoietin, improve the sensitivity of erythropoietin (EPO) receptors, coordinate the production of red blood cells, reduce the level of hepcidin, increase the content and activity of transferrin receptors, promote the absorption and utilization of iron, and resist received well.

Roxadustat is a 4-hydroxyisoquinoline derivative, a small molecule HIF-PH enzyme inhibitor. HIF is an important transcription factor that adapts to changes in oxygen in the body. Under normal oxygen conditions, the HIF-PH enzyme will promote the degradation of HIF; when the body is hypoxic, the activity of the HIF-PH enzyme is inhibited, and the accumulation of HIF increases, thereby inducing the corresponding Gene expression, allowing the body to adapt to changes in hypoxia. Roxadustat inhibits the activity of HIF-PH enzyme under the condition that the body is not hypoxic, increases the accumulation of HIF, and then produces a corresponding physiological response.

Roxadustat is well tolerated and has few adverse reactions. Most of its adverse reactions in clinical studies are relatively mild, such as headache, back pain, fatigue and diarrhea.
