Brimonidine is a highly selective α2-receptor blocker, successfully developed by Allergan Company in the United States. It is an ophthalmic drug that can reduce intraocular pressure (IOP). The commercially available brimonidine tartrate eye drops are the tartaric acid form of brimonidine, which is a good ocular hypotensive drug, suitable for chronic treatment of patients with open-angle glaucoma and ocular hypertension, and can also be used for Increased intraocular pressure after argon laser. Brimonidine tartrate ophthalmic solution has a dual mechanism of action by reducing the production of aqueous humor and increasing the outflow of pigment and scleral fluid. After this product is instilled into the eyes, the onset of action is rapid, the peak value can be reached after 2 hours of use, and the effective time can be maintained for 12 hours. Only trace amounts of drugs can be detected in the blood. This product is metabolized in the liver in the first pass, and the metabolites are excreted in the urine through the kidneys. Systemic absorption and distribution are not affected, and no dose adjustment is required in the elderly. Compared with other alpha-blockers, brimonidine tartrate eye drops are less lipophilic, not easy to cross the blood-brain barrier, and will not produce sedation, hypotension and other central adverse reactions similar to clonidine. Brimonidine tartrate eye drops have a high degree of selectivity for α2-adrenergic receptors, and the ratio of affinity to α2/α1 receptors is more than 1000 times. Acts with α2 receptors, which can lead to a decrease in intraocular pressure. The effect of the same α1 receptor will lead to vasoconstriction, eyelid retraction or reduction and pupil dilation. Long-term application of this product will not cause rapid desensitization. In addition, compared with β-blockers, brimonidine tartrate ophthalmic solution has a slight effect on cardiovascular and has no effect on pulmonary function, so it can be applied to patients who are forbidden to use β-blockers. Furthermore, when β-blockers alone cannot achieve the purpose of treatment, this product can be added at the same time.

This product can reduce intraocular pressure in glaucoma patients with only minor cardiovascular effects. Clinical experiments show that this product is equivalent to 0.5% timolol. The two drugs were used separately, 2 times a day, 1 drop each time, and the average decrease in intraocular pressure when the peak was reached: 0.79-1.01kPa for this product, and 0.8-0.88kPa for 0.5% timolol. After 12 hours of administration, the average decrease in intraocular pressure was 0.49-0.67kPa and 0.79-0.88kPa, respectively. The instructions for brimonidine tartrate eye drops recommend a 3-times-daily regimen, and some clinical trials have shown no difference in efficacy between twice-daily and thrice-daily.
