Quinidine is an antiarrhythmic drug. Commonly used its sulfate but also useful hydrochloride and gluconate. Quinidine sulfate is a white crystalline powder, odorless, extremely bitter in taste, with a melting point of 175-176°C, easily soluble in water and slightly soluble in ethanol. Quinidine is an alkaloid extracted from the bark of Cinchonaledgeriana (Moens), a Rubiaceae plant. It has direct inhibitory effects on excitability and contractility, and has anticholinergic effects on the heart. It is suitable for the cardioversion and prevention of atrial fibrillation, atrial flutter, supraventricular and ventricular tachycardia, and the treatment and prevention of malignant premature beats. Quinidine is the dextrorotatory body of quinine, and the two have similar pharmacological properties, but the effect of quinidine on the heart is 5 to 10 times stronger than that of quinine. The gate narrows, preventing the influx of Na+, resulting in a decrease in the depolarization rate of phase 0 and a slowdown in conduction. Due to the slowing of phase 4 depolarization, myocardial autonomicity is reduced and ectopic rhythm points are inhibited. Slow down the 3-phase K+ outflow, thereby prolonging the refractory period. In addition, it inhibits myocardial contractility and has anti-parasympathetic and sympathetic effects. Quinidine blocks alpha receptors and dilates blood vessels, especially when intravenous injection can cause a sharp drop in blood pressure, so it cannot be administered intravenously.

The pharmacological effects of quinidine against arrhythmia are mainly to prolong the myocardial refractory period, reduce the stress, automaticity and conductivity of the myocardium, and be used to treat atrial or ventricular tachycardia, and to convert atrial flutter and atrial fibrillation. and prevent its recurrence. When using quinidine, due to different etiologies and individual differences, the therapeutic dose and tolerance of quinidine in different cases are also inconsistent. Hyperthyroidism, chronic atrial fibrillation, and significant cardiac enlargement are more tolerated. Long-term heart failure, bundle branch block, coronary heart disease tolerance is low, sometimes the general treatment dose will cause poisoning, although quinidine poisoning is related to the size of the dose, but a small amount of individual cases can lead to arrhythmia. At present, it is believed that the inhibitory effect of quinidine on conduction can induce the reentrant excitation of the diseased myocardium. In order to reduce quinidine poisoning, the changes of heart rate, rhythm, blood pressure and electrocardiogram must be strictly observed during the medication.
