Vandetanib is a small molecule multi-target tyrosine kinase inhibitor developed by AstraZeneca. It was approved by the US FDA in April 2011 under the trade name of Zactima. This medicine is a tablet for the treatment of advanced medullary thyroid cancer in adult patients. Vandetanib is a multi-target tyrosine kinase inhibitor, belonging to the aniline quinazoline class of compounds, known as "second-generation Iressa", which not only acts on the EGFR, VEGFR and RET tyrosine kinases of tumor cells , but also inhibits other tyrosine kinases as well as serine/threonine kinases. Vandetanib is the first approved treatment for medullary thyroid cancer for the treatment of unresectable, locally advanced or metastatic symptomatic or progressive medullary thyroid cancer. Results of a randomized, placebo-controlled clinical trial showed that vandetanib significantly delayed the time to progression in locally advanced or metastatic medullary thyroid cancer. The recommended daily dose is 300 mg (oral), and treatment should be discontinued when the patient develops tolerance to the drug or cannot tolerate its toxicity. The most common adverse reactions to the drug were diarrhea, rash, acne, nausea, high blood pressure, headache, fatigue, decreased appetite, and abdominal pain. The incidence of adverse reactions is related to the dose. When <300mg/d, the patient tolerated it well, and the maximum tolerated dose (MTD) was 300mg. Phase II clinical studies involve many diseases. Clinical trials of vandetanib in the treatment of NSCLC are currently underway in my country. British AstraZeneca Pharmaceutical Co., Ltd. (AstraZeneca) obtained the world patent rights in May 2001 and October 2001 respectively (WO2001032651, WO2001074360), respectively, the structural formula of the compound, the synthesis method and the pharmaceutical composition (including the addition salt) ) and applied for corresponding protection.
